Important information and disclaimer

This page is for general education. Teriparatide is a prescription medicine and must be selected, prescribed and monitored by an appropriately qualified healthcare professional. This page does not diagnose osteoporosis, recommend treatment, replace a registered product information leaflet, or provide personal injection or dosing instructions.

A vial or powder labelled ‘teriparatide’, ‘PTH’, ‘PTH 1-34’ or ‘research grade’ is not thereby equivalent to a registered medicine. Identity, exact sequence, peptide content, purity, sterility, formulation, stability and cold-chain history may be unknown. Registered products are finished medicines with defined manufacturing and delivery-device controls.

Severe allergic symptoms, collapse, persistent dizziness, chest symptoms, severe weakness, confusion or symptoms suggesting marked calcium disturbance require urgent medical assessment.

What is teriparatide?

Teriparatide is the recombinant 34-amino-acid N-terminal fragment of human parathyroid hormone, written rhPTH(1-34). It reproduces the biologically active portion of endogenous PTH. When administered intermittently as a regulated medicine, it can stimulate new bone formation and reduce fracture risk in selected people with osteoporosis.

Identity distinctions that prevent confusion

What is teriparatide approved or researched for?

Registered indications vary by product and jurisdiction. In South Africa, the TERATYDE professional information lists established osteoporosis in postmenopausal women, primary osteoporosis in men, and osteoporosis associated with sustained systemic glucocorticoid therapy in women and men at increased fracture risk.

Severe or established postmenopausal osteoporosis

Primary osteoporosis in men

Glucocorticoid-induced osteoporosis in people at increased fracture risk

Fracture-risk reduction and increases in bone mineral density in appropriately selected patients

A medicine being registered for a defined osteoporosis population does not mean it is appropriate for general anti-ageing, bodybuilding, fracture healing without osteoporosis, or unsupervised bone-density improvement.

Other potential uses—only if future research confirms them

Teriparatide has been studied in selected settings involving fracture healing, spinal fusion, medication-related osteonecrosis of the jaw and other bone-repair questions. Evidence is heterogeneous and these uses must not be presented as routinely approved merely because small trials, case series or biological rationale exist.

Future research may better define treatment sequencing, retreatment, combination strategies and outcomes in very-high-risk subgroups. Any such use remains product-, patient- and jurisdiction-specific.

How does teriparatide work?

Intermittent PTH signalling

Continuous excessive PTH exposure can favour bone resorption, but brief intermittent exposure produces a different signalling pattern. Regulated teriparatide treatment preferentially stimulates osteoblast activity and new bone formation, while remodelling also increases.

Changes in bone over time

Bone-formation markers generally rise early, followed by changes in resorption markers. Bone mineral density usually increases most clearly at the lumbar spine. Fracture benefit cannot be inferred from a single early blood marker or from symptoms alone.

Why treatment sequence matters

Bone gained during anabolic treatment can be lost after it stops. Clinical plans commonly consider subsequent antiresorptive therapy to preserve gains, but the correct sequence depends on the patient, prior therapy, fracture risk and local guidance.

What has evidence found?

Fracture Prevention Trial

The pivotal placebo-controlled programme in postmenopausal osteoporosis demonstrated reduced vertebral and non-vertebral fracture risk and increased bone mineral density. The trial and later surveillance established an extensive evidence base beyond short-term biomarker change.

Severe osteoporosis: VERO trial

In postmenopausal women with severe osteoporosis, the randomised VERO trial found fewer new vertebral and clinical fractures with teriparatide than with risedronate over 24 months. This supports use in selected high-risk patients but does not make one medicine universally preferable.

Glucocorticoid-induced osteoporosis

Active-comparator research reported greater increases in lumbar-spine bone mineral density with teriparatide than alendronate and fewer new vertebral fractures in the studied population. Treatment still requires assessment of calcium status, renal function, ongoing steroid exposure and alternative therapies.

Long-term safety surveillance

Rat studies produced dose- and duration-related osteosarcoma, creating important historical restrictions. Large human observational studies have not shown an increased osteosarcoma incidence, but product information continues to advise avoidance in people with elevated baseline osteosarcoma risk and careful control of treatment duration.

Evidence and regulatory position in 2026

Current South African status

Teriparatide is not merely an investigational peptide in South Africa. SAHPRA professional information documents TERATYDE as an S4 registered medicine, registration number 58/21.4/0003, first authorised on 22 July 2025. Its stated strength is 250 micrograms/mL in a prefilled pen delivering defined daily doses.

This status applies to the specific registered finished product and its approved indications. It does not legalise or validate independently imported bulk peptide, unregistered pens, compounded copies, online “research” vials or advertising beyond the registered professional information.

The South African product information sets a maximum total treatment duration of 24 months and says the course should not be repeated over a patient’s lifetime. Other jurisdictions may use differently worded duration provisions; the applicable local product information governs South African use.

How teriparatide compares with other osteoporosis medicines

Known risks, contraindications and uncertainties

Commonly reported effects include nausea, dizziness, headache, limb pain and injection-site reactions. Transient orthostatic hypotension can occur, especially early in treatment. Teriparatide can temporarily raise serum calcium and may increase urinary calcium.

Do not use in hypercalcaemia or hyperparathyroidism; causes of abnormal calcium or alkaline phosphatase should be investigated.

Avoid or contraindicate use, according to the applicable label, in skeletal malignancy or bone metastases, Paget disease, prior skeletal radiation and people with open epiphyses.

Active or recent kidney stones, significant renal impairment and digoxin therapy require particular clinical consideration.

Pregnancy, breastfeeding and paediatric use are excluded or discouraged in product information.

New severe hypersensitivity, calciphylaxis or worsening cutaneous calcification requires urgent professional review.

Treatment and measurement safeguards

This page deliberately does not convert micrograms to syringe units, describe reconstitution, or provide a regimen for bulk material. Registered teriparatide is supplied at a defined concentration in a validated pen. Recreating that dose from a powder introduces identity, concentration, sterility, stability and measurement risks.

Clinical monitoring may include fracture history, bone-density testing, serum calcium, renal function, vitamin D and evaluation for secondary causes of osteoporosis. Results must be interpreted in relation to timing, prior therapy and the registered product information.

Common questions

Is teriparatide the same as parathyroid hormone?

It is the recombinant active 1-34 fragment of human PTH, not full-length PTH and not an unspecified “PTH” powder.

Is teriparatide experimental?

No for its registered osteoporosis indications. It is an established prescription medicine, although some proposed uses remain investigational.

Does it make bones permanently stronger?

It can increase bone mass and reduce fractures in selected patients, but gains may decline after stopping; a long-term osteoporosis plan is important.

Can it be used for ordinary fracture healing?

Routine fracture-healing use is not established merely from biological plausibility or limited studies.

Does a CAS number prove a supplier product is equivalent?

No. A CAS reference does not prove sequence, peptide content, formulation, sterility, stability, device performance or regulatory approval.

Is it available in South Africa?

A SAHPRA professional-information record exists for TERATYDE, an S4 registered teriparatide medicine. Actual supply, price and reimbursement should be checked through lawful pharmacy and prescriber channels.
